Each year ALL athletes are required to complete a variety of forms to ensure that they are healthy and prepared for the rigors associated with extra-curricular sports. These are REQUIRED to participate in any practice or tryout for any sport. The first part is the sports physical, it must be completed by a medical professional (urgent care, some pharmacies, general doctors) and it must be completed on the form located in the forms section of the website. Please ensure that it includes the full medical history of the athlete on the front, along with the student signature and parent signature at the bottom. The medical professional that does the evaluation should stamp the back of the form with their facility information along with completing the check box that says "cleared" and signing in the box. The second part is the RankOne forms. The link is posted in the links section of the website. There are 3 required forms to be filled out: - UIL Signatures Page - Bona Fide Residence - Consent to Treat You can also attach copies of your athletes Birth certificate (for age verification) and Utility bill (for address verification) in RankOne. Completing these BEFORE the start of the school year will go a long ways towards ensuring your athlete does not fall behind in getting their paperwork completed and starting the year off on the right foot!
